Update minimum two years relevant Trade license with money receipt.02. BIN Certificate with Vat Honor Card and last three months Submission copy must submit.03. Income Tax Registration and last year tax payment certificate.04. Update Bank Solvency Certificate with statement from 01-07-2025 to previous day of submission.06. Membership Certificate from relevant business group07. Each items sample must submit before final submission08. Manufacturer Authorization letter for all items mandatory09. Technical Compliance sheet in company letter head pad as fer specific e-gp format 10. Manufacturer quality certificate GMP/ISO/DGDA Certificate. MRP list must be highlighted with a marker pan and then uploaded with offer.11. Affidavit for proprietorship by 1st Class magistrate/notary public or incorporation certificate in case of limited company. 12. Undertaken in company letter head pad that there is no litigation and not blacklisting to any others institutes 13. Declaration on each item shall have batch wise quality control certificate and packet/strip/ampule/vial shall bear the words "Government Property, Not for Sale" and shall have a red and green wrapper.
